How to Apply for a Supplemental Ancestor
Once your initial application has been approved and you have your member number, you are welcomed to submit as many supplementals as you want.
- Request an application for a supplemental from the membership chair at OFFNCMembership@yahoo.com
- Prepare an application form with the new supplemental ancestor on page 1.
- Enter verbatim from your initial approved application in the proof boxes, with any corrections/changes the Genealogist may have made, and follow that by: OFFNC Member #, that number being your member number.
- Repeat this step up to the generation that is a new person. At that point, provide proof from primary sources of how that and subsequent generations connect.
- Include a check made out to OFFNC in the amount of $60 for each supplemental submitted.
